We hereby apply for membership in the Plumbing-Heating-Cooling Contractors Association of Eastern Ohio.

We agree to abide by the Constitution and By-Laws of the Association, all rules and regulations now in force, and those, which may be regularly adopted by the Association.

We understand that this Membership includes the right to use all Association and affiliation identification, and upon cessation of our Membership at any time, we will discontinue the use of this identification at once.

It also includes affiliation with the Ohio State Association and the National Association of Plumbing-Heating-Cooling Contractors.
